Use this tab to record the details of an incident that involved harm to a person.
Note: This tab only becomes available if this is flagged as a safety incident when the incident is added.
Use this subtab to record the nature of the harm sustained by the employee, the part of the body affected and the level of treatment given. The details entered here are required for all accidents involving harm.
This is an optional additional comment about treatment required.
Select the option button that indicates the level of treatment given to the affected employee. This is required for Accident reports.
Tick the check boxes for all of the body parts affected. This information is required for accident reports.
Tick all the check boxes applicable to this incident. This information is required for accident reports.
Use this subtab to record the circumstances of a serious harm incident or accident. The details entered here are required for all accidents.
For all accidents, select the mechanism from the options available. Mechanism describes a category for this accident type specified by OSH for statistical reporting purposes.
The Agency of Accident is required for all accident reports for OSH statistical purposes. Select the agency that best describes the outside force that contributed to the accident. Note that this is not the cause of the accident.
Use this subtab to enter the details of a serious harm incident or accident that will used in a report to OSH.
This describes where and how the accident happened. It is included in the Serious Harm Accident notification to OSH so it should be concise and accurate.
Has an investigation been carried out?
Select the Yes option button if an investigation into the accident has been carried out.
Was a significant hazard involved?
Select the Yes option button if the accident involved or was caused by a significant hazard.
The name of the person completing the report on behalf of the company.
Job title of person completing report.
Date of completion of Serious Harm Notification or non-serious harm accident report.
Select the option button that describes the role within the organisation of the person completing the reporting requirements.
Serious Harm Notification subtab
Use this subtab to enter the details of a serious harm or a non-serious harm incident or accident that will used or for internal purposes.
The date OSH was notified (note this is not usually the date the report was sent). In the case of serious harm accidents OSH must be notified as soon as possible after the accident by phone or fax. At this time OSH will say whether or not the accident scene needs to be frozen, etc.
The name of the person who notified OSH of the serious harm accident.
The date the completed report was sent to OSH Services at the Department of Labour.
The name of the person who actually sent the report to OSH.
For a serious accident, select "Yes" or "No". This is a requirement in the case of a serious accident.
Authorised to Disturb Accident Scene?
For a serious accident, select "Yes" or "No". An authorised person at OSH may give a dispensation to disturb the scene in situations where there may be significant disruption otherwise.
Specify the name of the person at OSH who gave the dispensation to disturb a serious accident scene.
