To access this tab: Click the Incidents icon
on the toolbar, then click Add or Change. Select the Investigation tab.
Use this tab to enter the details of an investigation carried out internally into an incident or accident. These investigation details are not included in the accident notice to OSH.
The full name of the person responsible for conducting the investigation.
The date to indicate when an investigation started. An investigation may take hours, days or even months to complete.
The date the investigation is completed.
Used to enter a summary finding of the investigation. Should include causes and recommendations for improvement, if required.
Used to add an element of the investigation and findings. For example, the location, the injured person, the task they were doing at the time, witnesses, the time, etc. Each of these elements is then investigated separately to see what facts come to light.
Used to edit and update the details of an element of the investigation.
Used to delete an element of the investigation.
